AACOMAS Fee waiver application

Hi guys,

I was interested in applying for the fee assistance program for AACOMAS (the application service for DO schools) and was wondering if you only need to put your own income (assuming that you filed your tax as an independent individual) or if you need your parents' income as well.

Fromthis page it seems like you only need to enter your income, but I am not sure since I can't see what the application looks like at the moment.
